It is important to document the reproductive health training you have received in residency and post residency work. Develop a file with all CME and other training certificates.

This file should include:

I. Professional Credentials

• Copies of degrees from medical school residency, and fellowship (if applicable)
• Evidence of Board Certification
• Controlled Substances Registration Certificate
• List of professional references
• Professional Memberships and Credentials
    - AAFP
    - STFM
    - NAF
    - other professional organizations

II. Knowledge and Training: Abortion Care

• Documentation of Training, including Program Curriculum and Training Log (see Chapter 10 for Training Documentation tools)
• Documentation of any other training, e.g. ultrasound training
• Letter from abortion training program director stating competency
• Copies of all CME certificates

III. Updated copy of CV
